Name and Address Change for Child Support Payments

If your name or address has changed, complete the Personal Information form, and mail it to the appropriate agency listed below.

For Department of Revenue child support cases, both the Department of Revenue Child Support Enforcement Office and the Clerk's Office must be notified.

For non-Department of Revenue child support cases, only the Clerk must be notified.
